Notice: Centre for Women, Family & Child Health

The Ministry of Health & Wellness has been made aware that a number of patients have been experiencing difficulty in contacting the Centre for Women, Family & Child Health at Caribbean Plaza on West Bay Rd.  Patients who need access to their personal medical records being held at this facility are asked to email appointments@forhealth.ky directly to request said records.

In an effort to safeguard patients, current patients of the Centre for Women, Family & Child Health who are unable to access their records or are unsuccessful reaching the facility directly, and who need assistance with the continuity of their care can request guidance from the Ministry of Health & Wellness by completing this form: https://app.smartsheet.eu/b/form/84da65ba202547daaa7cf2ddafaf41f2 , which can also be accessed via this QR Code.

Those who cannot access the form can contact MOHHELP@gov.ky  with the patient’s name and contact information only (no medical information please) so that they can be contacted directly, or call 936-8612 Monday to Friday, 9am-7pm until Tuesday, 30 September 2024.

Patients in the following categories are particularly encouraged to reach out:

1. Patients in the process of being investigated for possible illnesses

2. Patients who are awaiting the results of screening tests

3. Patients who are in the process of being treated by this clinic

4. Pregnant women who have booked their antenatal care and deliveries to be undertaken by clinicians from this clinic.

Alternatively, patients from this clinic are encouraged to seek services from another licensed provider to ensure continuity of care.
